Im not sure which mega drive you want, I used a breakout instead. there are multiplier ways to connect the wiikey to the gamecube I believe ipl 1.1 and later work fine. I have mine hooked up to a rev c gamecube and it works.

yes the heatsink and fan are both required. you can find replacements. some people use laptop heatsinks or different smaller fans. or trim the heatsinks. also the portable needs some kind of air flow a simple intake and exhaust vent will do.
all the broken ones I see are still $50 at least usually around $100 what I have seen done is using wiiu gamepad cases and frankencasing them into a proper case as seen here

only use the 5v if you want rumble, that is all it is there for. if you want to save power leave it out or put in a switch to turn it off. I don't know if having 2 controllers will make a huge difference in power consumption but I doubt having a switch on 3.3 and data will matter at all.
