Are you satisfied with your provider? - Not really. 3G speeds are slow as snot, and they never did anything with the 4G WiMax. I have a 4G phone that might as well not even have the 4g radio.

(I'm using a crappy Android phone until I get back to the states, my wife is using my Fascinate. I'll list how I had it set up/would set it up now.)

What ROM are you using - I had Cyanogenmod 7, would probably be using whatever 4.x is stable

How do you like it - I like it, but battery life could be pretty terrible at times.

Do you plan on upgrading any time soon - Yup, early next year. Thinking a Nexus 4.

Are you satisfied with your provider? - No, they're assholes that charge an exhorbitant amount of money for anything, and charge for incoming SMS, which shouldn't even be legal because it's not like I can reject a text message.

On a side note, I'm surprised TRH, I had you pegged as an iPhone owner.

Even if you don't open it? If you don't open it, I don't think they can legally charge you.

Currently paying $45/mo for unlimited everything on Straight Talk, which resells AT&T and T-Mobile's service. Using an AT&T SIM right now, but I'll probably order a T-Mobile one soon so I can get the full HSPA+ speeds out of my Nexus 4. Overall, I'm really happy with them though. Same exact service for a fraction of the cost.

Are you satisfied with your provider? - So far. LTE coverage is good in my area, but it eats my wife and I's 4gb of shared data pretty quickly. I plan on using wifi as much as possible to save the mobile data for when it's needed.

T-Mobile is fantastic here now, they've gotten so much nicer after the failed AT&T buyout. Especially since they launched their LTE network, it's the fastest one I've tried.

However, on my roadtrip last summer I noticed coverage was super spotty in rural areas away from major cities. Even at it's worse though I was still able to make calls and such, but in some areas data was basically non-existent (or you were roaming on AT&T).

It's definitely the best provider imo if you live in or near a major city, but otherwise it really depends on what the reception is like.
 
There's no service to speak of around here. They sell it and all, but 90% of the time, if somebody in my stomping ground doesn't have service, they're on T-mobile. My school was a complete dead zone for them. And I don't think they even have true 4g around here, just HSPA+.

I shall stick with Big Red (and rather orwellian) and reliable ~40mbit/s service for now.

Are you satisfied with your provider? - If they didn't lock down my phone and charge my father $200 a month for 3GB split across three lines, I'd feel that they're perfect. School reception is screwy, particularly in the places with especially slow Wifi (where I would be specifically looking for LTE, but can only get CDMA 3G/1X), but that's my school's fault
